Advancing your clinical delivery model through innovative approaches to transition care, care teams, clinical integration and patient engagement is the focus of this session. It is an in-depth study of how to transition from a siloed, traditional approach to comprehensive care utilizing multiple resources available as part of a CVSL. As we develop our CVSL approach, we have the opportunity to engage in new resources that will allow us to create a stronger continuity of care for our patients, resulting in improved outcomes. We will explore innovative models and opportunities to identify and fill in the gaps of our care continuum.
