Employee Award
This award recognises the dedication and contribution of an individual employee in delivery of services to older people and people with a disability.
The Employee will have:
- Excelled in the care provided
- Demonstrated commitment and dedication
In your submission please:
- Outline the Employees role in your organisation
- Describe how the Employee has excelled to care provision
- Describe how the Employee's commitment and dedication has been reflected in practice
- Supply any other information you consider relevant to this nomination
- Include a photo and up to 120 words of your choice to be used for an announcement of your nomination.
All nominees must complete both the Nomination Form & the Criteria Form.
