Bio

Gary E. Heyamoto, DDS, is a 1980 graduate of the University of Washington School of Dentistry, is in full time general practice (Bothell Washington). He has been recognized professionally with Fellowships in the International College of Dentists, American College of Dentists, Pierre Fauchard Society and the Academy of Dentists International; along with a Mastership in the AGD and two Lifelong Learning and Service Recognitions. Currently, he is serving on the WSDA's Board of Directors and as CDE Chair of WAGD.

He has been an active participant in identifying human remains in Snohomish County and has participated in several mass disaster drills as member of the Washington State Mass Disaster Team. Outside of dentistry, Dr. Heyamoto is a team statistician for the Seattle Seahawks and the UW Husky Football and Basketball teams.

Course Cancellation Policy:

Full refunds will be granted if written requests are received by the AGD by June 13, 2016. Cancellation requests received less than 30 days prior to the first official day of the annual meeting, with the exception of AGD member dentist registrants who have been elevated to delegate or alternate delegate status, will not be eligible for a credit or refund.
