The Leukemia & Lymphoma Society (LLS), provider number #1105 is approved as a provider for social work continuing education by the Association of Social Work Boards (ASWB) www.aswb.org. Approved Continuing Education Program (ACE). Approval Period: 12/10/2017 - 12/10/2020. LLS maintains responsibility for the program. Social workers should contact their regulatory board to determine course approval. Social workers will receive 7.5 CE clinical contact hours.

Instructions for Credit
There is no fee for this educational activity. To receive credit for this CME/CPE/CE activity, complete the preassessment, course, post-assessment, and evaluation and return it to the on-site coordinator. Your certificate of credit will be e-mailed to you within 4 weeks. For pharmacists, MLI will accept your completed evaluation form for up to 30 days and will report your participation to the NABP only if you provide your NABP e-Profile number and date of birth.
