PHYSICIAN CREDIT DESIGNATION
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Medical Learning Institute, Inc. and The Leukemia & Lymphoma Society. The Medical Learning Institute, Inc. is accredited by the ACCME to provide continuing medical education for physicians. The Medical Learning Institute, Inc. designates this live educational activity for a maximum of 2.0 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

REGISTERED NURSING DESIGNATION
Approval for nurses has been obtained by The Leukemia & Lymphoma Society National Office under provider number CEP 5832 to award 2.0 continuing education contact hours through the California Board of Registered Nursing.

MOC STATEMENT
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 2.0 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program.  Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.  Participation information will be shared with ABIM through the ACCME’s Program Reporting and Activity System (PARS). Individual or aggregated participant data may be shared with the funder of the activity.

INSTRUCTIONS FOR CREDIT
There is no fee for this activity. To receive credit for this CME/CE activity, complete the pre-assessment, course, post-assessment and evaluation form and return it to the on-site coordinator. Your certificate of credit will be emailed to you within 4 weeks or MOC credits reported to the ABIM within 4 weeks.
