Registration and Accommodation

Registration for the Consensus Conference is by invitation only. If you would like to be added to the invitation list for the 2015 Conference, please email WC5@buksa.com with your request.

The 2014 Conference took place at the Delta Bessborough Saskatoon in Saskatoon, Saskatchewan. 

Registration Fees 
Regular Registration: $250.00 plus 5% GST ($262.50 total)
 
Cancellation Policy
Notice of cancellation must be made in writing to the Conference Secretariat at WC5@buksa.com. A $60.00 ($63.00 with 5% GST) cancellation fee will apply for conference registrations cancelled until August 28, 2014. After this date, no refunds will be issued.

Consensus Development 
The objective of the Western Canadian Gastrointestinal Cancer Consensus Conference is to reach agreement on best practices in cancer management, using an evidence-based approach. At the same time, the conference is intended to provide educational and networking opportunities for the gastrointestinal oncology community across Western Canada. The consensus statement that comes out of the discussions at the conference can affect practice, and can serve as a tool for this community.

The creation of this consensus serves as justification for the provision of transportation and accommodation for all participants in exchange for a modest registration fee. To ensure the integrity of this meeting and the resulting consensus statement, it is imperative that all registrants participate actively in the discussions leading to consensus in the morning of Saturday, September 6, 2014.

For this reason, we will be noting attendance at the consensus discussions and charging an additional fee of $300 for those who are not in attendance for the Saturday consensus discussions. This notice is included in our registration policies and is agreed to by accepting them. Please note: This fee is not applicable to local attendees 
 
Registration Policies
  1. Registration for this meeting is by invitation only and must be completed using the online registration form.
  2. Participants are expected to attend the complete Conference including the consensus discussions. Those not present for the consensus discussion and decision will be charged an additional fee of $300.00. Please note, this fee is not applicable to local attendees.
  3. The fee for this conference is $250 plus 5% GST ($262.50 in total). BUKSA Associates Inc. is not a GST exempt organization, GST will be charged on all registration fees. Registration fees are listed and must be paid in Canadian Dollars (CAD).
  4. Invoicing will not be available for registration fees. These must be paid at the time of registration using either a VISA or MasterCard. Your statement will read “Buksa Assoc. (780) 436-0983 Edm.”
  5. Your registration includes:
    1. Airfare: Return airfare at the lowest available economy airfare from your home airport to the Saskatoon John G. Diefenbaker International Airport. Bookings must be made through the official travel agent of the conference, Mick Morrison of Vacations Plus, Edmonton. Bookings made independent of the official travel agent will not be reimbursed
    2. Ground Transportation: Transportation to and from the Saskatoon John G. Diefenbaker International Airport will be supplied by pre-arranged shuttle.
    3. Accommodation: Two (2) nights’ accommodation for one (room is double occupancy) guest at The Delta Bessborough will be provided. Additional room nights requested by delegates beyond the two provided will be at the delegate’s expense (see accommodation policies for more detail).
    4. Parking: Onsite parking for those travelling to the conference by car.
    5. Meals: All conference meals including two breakfasts, one lunch, and one dinner.
  6. Your registration does not include:
    1. Parking at your home airport or mileage to and from your home airport.
    2. Meals taken independently from the conference.
    3. Vehicles rented by delegates to attend the conference.
  7. Advance registrations will be accepted until September 3, 2014. After September 3, 2014, if space remains available, registrations will only be accepted on-site.
  8. Substitutions may be made until September 3, 2014 by writing to the Conference Secretariat at WC5@buksa.com or on-site.
  9. A confirmation notice detailing your registration and a receipt will be sent to you immediately after completing this form. Please ensure that your email filter is set to accept emails from WC5@buksa.com for the purpose of conference communication.
