You do not need to turn in your eye exam prior to attending the class. However, we will hold your certificate until you provide proof of your eye exam within the past 12 months. The eye exam must be administered by an optometrist, medical doctor, registered nurse or certified physician's assistant. We accept the form from their offices, but if you need, see sample form here.
