$100.00 registration fee when registering on the website by February 28, 2017

$125.00 registration fee when registering on the website between March 1 and March 6

On-Line Registration closes at midnight on March 6, 2017 

ON-SITE REGISTRATION

On-site registrations is $150 and will be taken on a space available basis.
Be sure to view the "Speaker Presentations" page and  the Agenda prior to the program. 

Refund Policy 

Refunds granted until March 6, 2017, no refunds after that date. 

If you paid by credit card, registration fees will be refunded back to the credit card used to register, minus a $25 processing fee. E-mail Wendy.L.Friedman@kp.org  with your cancellation request.               If symposium is canceled, the above refund policy will be enforced.
