Connecting LTC providers through technology - Bridging multiple and distant Healthcare Specialists

Residents today need more extensive support than ever with 1 in 3 residents dependent on care staff. Moreover, 41% of LTC residents are hospitalized at least once in the 6 months before their death. This growing complexity, combined with an increasingly overburdened and dispersed collection of specialists, has heightened interest in “virtual” care options as a means to spread knowledge and improve care. This workshop will demonstrate proof-of-concept virtual care technologies as a means to explore known treatment workflows and identify new clinical use cases, across multiple stakeholders (pharmacists, nurses, physicians, PSWs, etc). Through structured dialogue, the goal of this workshop is to gather front line input regarding Long Term Care specific needs: Which clinical episodes can virtual care technology address most readily? Who would benefit the most (in the circle of care)? What adoption barriers exist in the sector? Where and how do residents / families fit in? Beyond clinical research and leading-edge technology, what will make virtual care solutions a success in your home?
