Agitation and aggression affect up to 80% of patients with Alzheimer's dementia (AD-AA). Current treatments have only modest efficacy and are associated with several adverse effects and increased mortality. To address these concerns and improve the treatment of AD-AA, we designed and implemented an Integrated Care Pathway (ICP) at CAMH geriatric psychiatry inpatient unit. The ICP is a step-by-step algorithm of behavioural and pharmacological interventions guided by structured assessments. We had 45 patients with AD-AA undergo the ICP. 42 patients completed the ICP successfully. Polypharmacy rate was 2.4% as compared to near 50% in the community. There was 42% reduction in rate of falls as compared to usual care. After successful ICP implementation at CAMH, we are now rolling it out to long-term care sector.
