Public Health Ontario (PHO) has received a number of requests for support to address the overuse of antibiotics for presumed urinary tract infections (UTIs) and associated antibiotic-related harms. A multi-component program was developed to address barriers to practice change that have been noted by leaders in long-term care that are working to make improvements in this area. The program focuses on providing supports to address 5 practice changes that collectively will help improve decision-making processes around the use of urinary antibiotics. Nine implementation strategies are outlined that can be tailored to address barriers to practice change associated with obtaining buy-in and support; increasing knowledge amongst staff, consultants, prescribers, families, and residents; and monitoring and supporting practice change. This presentation will share what has been learned from piloting this program in 12 LTCHs across Ontario.
