Heart Failure (HF) affects 20% of LTC residents. We previously showed the feasibility of an intervention to: (1) improve staff knowledge; (2) develop HF-specific IP processes; and (3) integrate these learnings into existing LTC processes. 

A retrospective audit of the intervention was carried out on two neighbourhoods of one Ontario LTC home. A team of RPNs and PSWs was trained to assess residents for possible HF using simple and validated criteria, and implement IP communication protocols. Specialist-mentors conducted biweekly bedside teaching with the team. 

Fifteen residents (mean age 88.5 years) were assessed from December 16 2016 and March 15 2017; HF was confirmed in 10. Treatment recommendations were made for all residents, leading to a significant reduction in symptoms. 

An interprofessional education intervention facilitates the identification of LTC residents with HF, treatment modifications and symptom relief. These findings justify expansion of the intervention to other neighbourhoods and LTC homes.
