The complexity and frailty of the residents served in the long-term care sector is increasing. Healthcare providers and students require clinical resources and tools to bridge the knowledge to practice gap to ensure competence in communicating vague changes in condition in the older adult with frailty that are associated with unnecessary emergency transfers to acute care settings. The Sensory Observation System (SOS) Checklist App improves healthcare providers’ ability to recognize early changes in condition and supports the team’s response and decision-making through systematic organization of observations and assessments. The app also supports family members to describe changes in their loved one. This app generates a tailored SBAR (Situation-Background-Action/Assessment-Recommendation/Request) report that can be used for documentation and further communication with the team. This poster will share the app design and development. Continuous quality improvement efforts and partnerships will be described. Beta testing results related to usability and functionality will be shared.
