The Long-Term Care Best Practices Program is funded by the Ministry of Health and Long-Term Care to foster the development of evidence-based practice cultures in long-term care homes (LTCH) using RNAO’s clinical and healthy work environment best practice guidelines and other evidence-based resources. Fifteen Long-Term Care Best Practice Co-ordinators expert in knowledge translation and transfer are located across the province. A coaching, facilitation and consultation approach is used to engage LTCH leaders, nurse practitioners and direct care staff to adopt evidence-based practices that support consistent approaches to providing high quality care to residents and families. This presentation will highlight the six strategies used by the program. Results of a program evaluation including a provincial survey of LTCHs determined how RNAO and other evidence-based resources were used and their impact on clinical practice, resident outcomes and the supports needed to implement and sustain evidence-based practices will be shared.
