Health Quality Ontario and the long-term care sector are working together to spark improvement by connecting clinicians, care teams and researchers through a developing Community of Practice.  This long-term care Community of Practice is a network where people working in the sector can discuss best practices, share local innovations and foster knowledge exchange to drive quality improvement and improve resident experience.  

The long-term care Community of Practice connects the sector through an on-line platform (discussion forums, document library, tools and resources etc.), webinars, Quality Improvement Plan training and face-to-face workshops at provincial long-term care conferences.

This poster will focus on the Community of Practice’s development and its benefits, while highlighting how this community can facilitate the use of Quality Improvement Plans, Practice Reports and Quality Standards.
