Communication within the interdisciplinary team in each Long Term Care home has never been more crucial. Residents experience multiple high risk areas such as falls, skin breakdown, incontinence, pain and others that affect their quality of life. Effective communication can be challenging to ensure that each discipline proactively identifies areas of concern and provides input into a comprehensive plan of care. Residents and families trust that care providers are knowledgeable about their needs and assist in achieving the best outcomes.

 Faced with this challenge, Kensington Gardens, home to 350 residents, implemented monthly high-risk meetings on each RHA to review each resident whose level of function has changed. Using a tracking form, the team examines high-risk areas; RAI-MDS outcome scales scores and current interventions in place and their effectiveness. This model helps provide consistent communication to all team members to better meet the residents’ needs and improve their quality of life.
