Introduction:
Oncocytic changes within a pleomorphic adenoma may be worrisome for malignant changes and may result in a misdiagnosis.  We present a case report of this rare benign parotid neoplasm.

Case report:
A 70-year-old female presented with a left parotid tumour which has been increasing in size over the past few years.  She underwent a left superficial parotidectomy and was diagnosed with oncocytic variant of pleomorphic adenoma.

Conclusion:
Oncocytic variant of pleomorphic adenoma is an uncommon benign parotid tumour and clinicians should have a high index of suspicion when assessing parotid neoplasms. Clinically, it behaves similarly to a pleomorphic adenoma and treatment consists of complete surgical excision with clear margins.
