Nephropathy in Pregnancy

Background: A 27 year-old primigravida presented with
haematuria and proteinuria at five weeks of gestation on a background of
recurrent urinary tract infections in childhood and a previous episode of
pyelonephritis.
Case Report: At 23 weeks of gestation, the serum creatinine
level was found to be 450 mmol/L with nephrotic range proteinuria (4 g/day);
clinically consistent with a rapidly progressive glomerulonephritis (RPGN). She
was referred to our tertiary referral maternity hospital and renal biopsy
demonstrated crescentic immunoglobulin A nephropathy. She was commenced on
dialysis and administered pulse intravenous methylprednisolone and oral
cyclophosphamide. Delivery was advised at 26 weeks due to deterioration in the
foetal condition on doppler ultrasound. Post-partum, the patient was no longer
dialysis-dependent, however serum creatinine was in the order of 400 and she
was discharged with ongoing treatment.
Conclusions: Renal biopsy and diagnosis of an acute RPGN in
pregnancy is rare.
