A 35 year old lactating female presented to the Emergency Department with a two to three day history of vomiting and was found to have a severe metabolic acidosis with pH of 6.92. After exclusion of other causes of metabolic acidosis, it was determined that the patient had a severe starvation ketosis with the lactating state having contributed to the acuity of the presentation. A review of associated literature revealed a very limited number of similar case studies. We present the case here to highlight a potentially underappreciated cause of non-diabetic ketoacidosis.
