ABSTRACT

Title: Transitioning From Paediatric To Adult Care

Transitioning care from paediatric to adult services is problematic for both patient and parents and indeed doctors.

In the case of renal transplantation the period from 16-26 years of age is associated with high risk of transplantation loss due to non adherence to immunosuppressive medications. Up to a third of renal transplants in this population can be lost due to non adherence to medication.

Groups in Colorado and Oxford have shown that integrated transition (14-18 years) and young adult clinic (18-26 years) can achieve longterm graft loss to below 3%.

We will present data on the changes in graft loss, adherence, quality of life after the initiation of integrated transition and young adult renal clinics in Adelaide
