9D: (Patient Pathways) Mission Possible: Integrated Care Management for Chronic Patients

Abstract:
New payment and care delivery models are changing how hospitals and health systems address the complex needs of chronically ill patient populations across the care continuum. Integrating care for kidney patients is both a significant need and opportunity given that these patients account for 36% of hospital readmission and 10% of Medicare costs. Whether you’re readying to become an ACO, focusing on reducing readmissions, or implementing bundled payments, today’s healthcare environment is demanding care managers coordinate care in and between care settings, while demonstrating a positive return on investment. During this session our Special Forces team will share practical examples of administrative and clinical interventions and practices that can deliver a real return on investment and improve the patient experience.
