  • Session Abstract:
    Healthcare Reform requires significant reengineering of existing case management programs. With the advent of Accountable Care Organizations, the focus must shift to supporting integrated partnerships among key stakeholders affecting quality patient care throughout the continuum. Hospital-based case managers need to work in close collaboration with patients/families, primary care physicians (medical homes), specialists, post-acute care providers, and continuum of care case managers to ensure quality, accountability and efficiency as patients transition from one care settings to the next. This session will focus on how you can use the Morrisey Concurrent Care Manager™ (MCCM™) web-based care management system to support all your required workflows and follow-up communications from pre-admission to post-discharge, including:

    - Preadmission reviews to evaluate the appropriateness of scheduled procedures and to complete initial patient risk assessments

    - Emergency department case management to support patient assessment and collaboration with physicians to identify appropriate level of care assignment

    - Inpatient reviews to evaluate medical necessity, communicate with payors, identify and manage discharge plans, track placement barriers, avoidable delays and interactions with both internal and external physician advisors, etc.

    - Readmission reviews to document findings and reasons along with interventions to help prevent future unexpected readmissions

    - Care transitions management, including discharge planning and coordination of services with post-acute care providers

    - Continuum of care case management to ensure smooth transitions between inpatient and outpatient case managers

    This session will follow a patient-focused case study to demonstrate use of MCCM’s automated case management workflows, rules and work listing functions. The goal of the session is to demonstrate a successfully integrated case management program designed to meet the challenges of Healthcare Reform.
