Abstract:  Case Managers at the University of Kansas Hospital (KU) faced challenges in providing care for their undocumented patients. This session will illustrate how Case Management staff at KU work with MexCare to develop a timely and cost effective plan to safely transition these patients to Latin American facilities near their homes and families.

1.  Identify barriers to discharge for undocumented patients

2.  Understand best practices used by the Case Management staff at KU to help identify, transfer, discharge and place their undocumented patients

3.  Learn how to work with Mexcare to create a collaborative transition plan to ensure timely and cost effective patient discharge, transportation to and admission in appropriate Latin American facilities

4.  Reduce length of stay while maintaining quality care in the patient’s home country
