Description

ACMA’s Social Work Fellowship is a nine-month hospital-based program, which offers a premiere educational experience for social work professionals desiring to expand their understanding of hospital social work and case management practice. In this session, the 2012-2013 Fellow will provide an in-depth SWOT analysis of the case management services at the Fellowship site, UCLA Health System, and two additional case management models observed by the Fellow during the program: Mayo Clinic Hospital and Presbyterian Intercommunity Hospital, winner of the 2012 Franklin Award of Distinction.  The Fellow will examine these three different models for case management, compare and contrast the services provided, and share her experiences and lessons learned during the program.

LEARNING OBJECTIVES:

1.       Compare and contrast three different case management models

2.       Understand how the Fellowship program further promotes social work in the hospital/clinical setting and raises awareness regarding social work's key role in Hospital Case Management
3.     Apply concepts and processes from the models discussed to their own case management program
