Description
This session will explore one organization’s development of a disparities dashboard, designed to monitor and address the equitable provision of care regardless of race or ethnicity by providing a snapshot of race, ethnicity and language (REaL) needs as well as Appropriate Care Scores for the following patient populations: AMI, HF, CAP, and SCIP; 30-day readmission, and patient satisfaction. The dashboard’s implementation process and the national call that prompted its creation will be examined. The speakers will discuss how the data from the dashboard can significantly enhance the organization’s capacity to monitor progress across race, ethnicity, and/or language services, improve patient outcomes, and enhance the quality of care it provides.
