5B: Care Plus Initiative Provides Uninsured Patients Access to Medical Services

Description

This session will examine the development of one organization’s Medicaid Waiver (Care Plus) program, designed to provide 20,000 uninsured patients access to an array of medical services. In addition to the set of medical services, the organization has employed onsite outpatient care coordinators who provide case management to patients in the program. The initiative focuses on patients with diabetes, heart failure and hypertension in addition to providing support to those patients identified as complex, requiring frequent hospitalizations. Presenters will discuss how case managers are the driving force in this initiative; improving the patient experience and reducing consumption of ED and hospital resources. The session will also examine the program’s outcomes to date.
