Data is a key part of the case management professional’s toolbox. Critical outcome measures such as readmissions and length of stay can be directly impacted by effective case management and many organizations hold case management accountable for these metrics. With so much accountability for crucial measures, it is imperative that case managers not only understand the data they are responsible for, but also understand how to present data and make it work in their favor. This interactive, two-part session will examine the case manager’s role in analyzing and presenting outcomes and critical data to hospital leaders, as well as other members of the care team. Essential elements of dashboards, scorecards and other tools used to convey case management’s contributions and impact will also be discussed.
