Description

This session will explore how one hospital case management team is expanding its outreach in the community by partnering with community emergency services through the local fire department. The presentation will outline the steps involved in establishing focused interventions directed toward improving community health, and reducing unnecessary emergency responses by EMS. The speakers will present the program’s outcomes and discuss case studies.

 

Learning Objectives:
1. Investigate EMS/ED access and develop collaborative relationships with community resources to identify key stake holders for effective program collaboration

2. Evaluate financial impact of interventions as evidenced in cost avoidance studies, demonstrating significant financial benefits by reducing unreimbursed ED/EMS contacts
3. Analyze ED/EMS patient referrals to determine case management interventions and referrals to community resources for development of care plans/protocols
