Description
Health Care professionals have recognized that silos must be eliminated and bridges built in their place to meet the growing challenges of healthcare delivery, both now and in the future. This is apparent in providing medical care, but also in meeting the needs of mental health patients. This session will examine two community hospitals that have accepted this reality and are working within their communities to collect data, and develop processes that provide care and ensure smooth transitions for this specific patient population. In this session, two distinct models will be explored. The first model includes five hospitals, law enforcement, the mental health system, a crisis center, homeless shelters and disability specialists. The second model connects uninsured and underinsured patients with a Federally-Qualified Health Center (FQHC) for psychiatric care in their medical home.
