4A: Building a Culture of Collaboration with Skilled Nursing Facilities

Description

This session will examine one organization’s collaborative efforts between care management and skilled nursing facilities (SNFs) within the community to implement a series of long term care initiatives focused on: facilitating effective sharing and communication of clinical information; developing and enhancing relationships and interaction between the hospital and skilled nursing facilities; providing ongoing educational opportunities to the skilled nursing facilities and reducing unplanned hospital readmissions. The speaker will discuss the program’s successes – including a reduction in 7 and 30-day readmission rates – as well as cultural challenges and barriers to effective collaboration.

1. Demonstrate the importance of working on a collaborative basis with SNFs to enhance the quality of care for patients

2. Identify strategies that acute hospitals and SNFs can implement to reduce unplanned readmissions

3. Discuss changes to work flow, which will improve communication of clinical information between facilities
