2A: HUG: A Simple Approach to Complex Cases

Description
This session will examine the High Utilizers Group (HUG), a process for multi-disciplinary review of complex cases that results in recommendations regarding the care of patients. The goal of the group is to improve treatment planning and maintain the patient safely in the community. The speaker will identify how the initiative began and discuss challenges and hurdles faced in the implementation process. Successes and outcomes will also be shared and case examples presented. The session will explore one strategy to effectively reduce unnecessary readmission rates in a high risk population.

1. Identify the effect of multi-disciplinary/multi-care level collaboration in reducing the risk of readmission
2. Describe effective interventions that aid in reducing the risk for patients with complex medical illness
3. Demonstrate how to positively reframe health care views regarding patients with complex illness
