This will be a panel presentation that will highlight and discuss national readmissions trends and benchmarks. The session will then present solutions from the panel members that have successfully improved readmissions in their organizations. These solutions will include discussion of how they were implemented, outcomes, impacted patient population, etc. The discussion will also address how to capture needed data, and how it is used and communicated to drive performance improvement.
