This presentation will focus on the continued bone loss in the posterior mandible when treating the edentulous mandible with dental implants. Using two independent implant overdentures (soft tissue-supported prosthesis) retains the prosthesis but does not preserve bone in the posterior mandible and does not support the prosthesis on the dental implants. This creates greater pressure in the posterior mandible, which can progress bone loss, often even faster than if the patient was unrestored or treated with a conventional complete mandible denture, due to disuse atrophy. To truly prevent bone loss and even increase bone volume and density, it is more viable to restore the mandible with four connected or joined implants to support and retain a fixed, fixed detachable or a removable prosthesis.

Objectives:
Identify posterior bone loss through disuse atrophy of the edentulous mandible with an implant-retained over denture.
Understand how to prevent posterior bone loss in the edentulous mandible.
Recognize proper prosthesis design of two vs. four implants to support a full mandibular prosthesis.
