This is a case study that follows a patient who complained of cold sensitivity and pain in the right quadrant. After a root canal treatment on No. 3 molar, followed by a build-up and crown restoration, she returned three years later complaining of pain described as an electrical shock that spread into the upper right quadrant, different than a regular toothache. Dental and neurological treatments reduced her pain level and frequency and lead to the diagnosis of SUNCT (short-lasting, unilateral, neuralgiform, conjunctive, tear headache).

Objectives:
Understand that a daily toothache may not be tooth pathology.
Recognize that different pain characteristics and distributions, including frequency and duration, could be a central nervous system issue (trigeminal nerve).
Understand that when a patient is numbed, but pain remains, the cause could be related to another medical discipline.
Understand that toothache is prodrome of headache.
