The OAFC annually recognizes the achievements of individuals and our partners. We invite you to review the following nomination opportunities and help us celebrate the dedication and hard work of fellow free clinic volunteers and partners.  Click the title of each award to download the nomination form.

This award was created by the Ohio Association of Free Clinics in 2003 to honor individuals whose dedication to and achievements in serving the healthcare needs of the working poor and uninsured provide outstanding service and leadership models. 
 
This award was created by the Ohio Association of Free Clinics in 2004 to honor corporations or businesses (both for profit and nonprofit) that have contributed significantly to either the OAFC or to free clinics. 
 

This award is presented by the Ohio Association of Free Clinics to recognize a member free clinic whose new innovation in patient care practice has led to significant improvement in their clinical practices in helping meet the healthcare needs of the working poor and uninsured.  It is our hope that this successful innovation will provide a model for other free clinics to follow.

This award is presented by the Ohio Association of Free Clinics (OAFC)  to recognize a member free clinic whose innovation in clinic administration has led to significant improvement in their ability to serve the healthcare needs of the working poor and uninsured.  It is our hope that this innovative practice will provide a model for other free clinics to follow.

Nominations must be received by 5:00 pm on August 31, 2012 by US mail, fax or email.
 
The OAFC expects to make its decision by September 12th so that arrangements can be made for the recipient to attend the 13th Annual Ohio Free Clinics Conference, October 8 & 9, 2012 at the Embassy Suites in Dublin. The awards will be presented at the Awards Luncheon on Tuesday, October 9th.
