Our program officially kicked off on Thursday morning, with Craig Abbott, CEO of Health Dimensions Group who welcomed everyone to the conference and introduced our first presenter Dr. Kathleen Griffin, who covered the new regulations and relationships with insurers and how providers are responding. Next up, Jerry Nye, Dr. Terry Carroll, and John DiCola discussed the various approaches being taken by leading organizations as they advance the development and implementation of their continuum of providers under health care reform.

Neal Peyser presented the results of the Health Dimensions Group national survey, revealing the current status and trajectory of post acute care. The HDG research represents opinions of more than 150 healthcare executives throughout the United States, and the fascinating results show that post acute services have emerged as a new, competing resource priority. Watch for a full white paper following the conference. 

The day ended with lunch on the Sunset Lawn as we enjoyed sharing and discussing topics during the lunch and learn sessions.

The final day of the Health Dimensions Group National Summit began with a power walk for those that awoke bright and early and everyone enjoyed a breakfast with sponsors.

Dr. Kathleen Griffin welcomed everyone back, and introduced the first session discussing technology in the care continuum. Doug Busch, Senior Vice President and Chief Operating Officer of Intel-GE Care Innovations and David Classen, Associate Professor of Medicine at the University of Utah led the discussion. The session included information about technology integration that can work to improve quality and reduce costs in acute and post-acute environments, as well as technology’s major impact in the coming years on the health care delivery system and the ability of patients to self-manage their conditions.

Following a refreshment break with sponsors, Ken Coburn, CEO and Medical Director of Health Quality Partners, addressed the guests in a session about Care Pathways Across the Continuum, which focused on what is required to ease patient transitions from one service or site of care to the next, among other related topics. 

The afternoon was then marked with a panel discussing the Post Acute Continuum within an ACO or Integrated Delivery System. The panelists included Janet Tomcavage, Vice President of Health Services at Geisinger Health Plan; Robert Palmer, Director of the Glennan Center for Geriatrics and Gerontology at the Eastern Virginia Medical School; Mark Enger, Vice President and COO of Care Delivery at Kaiser Permanente Northwest; and Audrey Weiner, President and CEO of Jewish Home Lifecare.
