$125.00 registration fee when registering on the website by May 1, 2019 at 11:45pm

$150.00 registration fee when registering on the website between May 1 and May 8

On-Line Registration closes at 11:45pm May 8, 2019

ON-SITE REGISTRATION

On-site registrations is $175 and will be taken on a space available basis.
Be sure to view the "Speaker Presentations" page and  the Agenda prior to the program. 

Refund Policy 

Refunds granted until May 8, 2019, no refunds after that date. 

If you paid by credit card, registration fees will be refunded back to the credit card used to register, minus a $25 processing fee. E-mail Wendy.L.Friedman@kp.org  with your cancellation request.               If symposium is canceled, the above refund policy will be enforced.
