*AYA Advocates include consumer representatives and those working for advocacy organisations who do not otherwise qualify for another category of registration.

NOTE: Healthcare Professionals and Health Service Executives and Managers who also act as AYA Advocates should register under their appropriate Healthcare Professional or Health Service Executive / Management Category.


** Anyone wishing to register as a student must be enrolled full time at a recognised educational institution. In order to qualify for this category, you must provide evidence of full time enrolment at the time of registration.

  • Morning tea, lunch, afternoon tea during congress
  • Welcome Reception on Tuesday 4 December, 2018
Day Registration includes:
  • Entry to all congress sessions and exhibition area on the selected day of attendance

  • Day Registration on Tuesday 4 December, 2018 includes Welcome Reception ticket
The congress Gala Dinner is not included within the registration fee and can be purchased separately via the online registration portal.

TERMS & CONDITIONS
PAYMENT

Registrations can be paid by credit card (Visa, Mastercard & AMEX are accepted). A 1.75% credit card surcharge (for Visa & Mastercard) and 2% credit card surcharge (for AMEX) applies to all credit card payments. You can also arrange payment via electronica funds transfer. No surcharge applies to pay via EFT but payment must be received within 14 days of your registration date. All prices quoted are in Australian dollars and include GST.

CANCELLATION

Cancellations received in writing to the Congress Organisers’ 45 days prior to the Congress commencement date will receive a refund less a processing fee of $120 including GST. No refund will be given after this date; however, a delegate substitution may be permitted. All cancellations and substitutions must be made in writing to the Congress Organisers. 

LIABILITY

In the event of industrial disruptions or natural disasters, Canteen Australia and Maxim's Travel accept no responsibility for any financial or other losses incurred by the delegate.

DISCLAIMER

The Congress program and registration information is correct at the time of publication. However, the Organisers reserve the right to change the information where necessary without notice.

PHOTOGRAPHY & RECORDING DISCLAIMER

During your attendance at the Congress, your image or voice may be recorded via video, photography or by any other means (“recorded”) by an official representative and this may be distributed or published at the discretion of the Congress Organisers’. If you do not wish to be recorded, you are required to formally advise the Organisers’ by emailing ayacongress@maxims-travel.com.

INSURANCE

Registration fees do not include insurance of any kind. Delegates are strongly advised to take out their own travel and medical insurance. The Organisers will not take responsibility for any participant failing to insure.
