The Pink Sari Project: A culture-centred approach to health communication, partnerships and sustainability

Presentation Summary: Most health initiatives targeting CALD consumers are direct translations of English language messages. The Pink Sari Project will describe how it moved away from that approach, co-created a campaign with communities, saw a 17% increase in mammograms with Indian/Sri Lankan women, and won an international award for best campaign.

Presentation Summary: The Lighthouse Hospital is a project here at Tamworth to "shine the light" on the issues around cardiovascular disease and the impact it has on the Aboriginal and Torres Strait Islander population. This message is told through powerful stories of key community members that have been affected by CHD.

Presentation Summary: We wanted to empower Aboriginal women with the knowledge and understanding of the signs and symptoms of heart attack. Currently the statistics are tragic we need to reduce the number of local women and their families affected by heart related deaths.

Presentation Summary: Patient-centered care contributes to patient satisfaction and better outcomes. The question is how to make the patient’s journey through the unfamiliar world of the operating theatre more patient-centred. A questionnaire was developed jointly with the consumer consultancy committee. 349 questionnaires were returned allowing for numerical and thematic analysis.

Session Title: Understanding the experience of women undergoing assessment with BreastScreen NSW

Presentation Summary: This presentation describes a research study conducted by the Cancer Institute NSW to understand women's experience of undergoing assessment with BreastScreen NSW. The study used a mixed methods design and the recommendations arising from the study will result in an improved assessment process for NSW women.

Presentation Summary: Bureau of Health Information (BHI) patient survey results show differences in experience of Aboriginal and non-Aboriginal patients in NSW, particularly for respect, kindness, trust, communication and discharge information. A descriptive analysis was conducted to determine if gaps in experience varied by setting and/or changed over time.

Presentation Summary: Consumers identified how easy or hard it was to access a health service via a trial methodology called First Impressions. The health service was keen to work closely with consumers to improve the environment. The methodology and outcomes are explored in this session.

Presenters will discuss how the initiative started, the momentum gained via extensive community partnerships & hopes for the future of the area.

Presentation Summary: The ID & IDA project utilised a redesign methodolgy approach to achieving a more patient-centred approach to better manage the identification and mangement of Iron deficiency in the primary care setting. This was achieved by better engagement with patients and carers through information provision and options targeted to identified needs.

Presentation Summary: Increasingly, consumers and funders are asking for more consumer input into clinical trial design, to minimise the burden to participants, recruit, and address the right questions. This workshop will explore participant and consumer rep experiences of clinical trials, with a view to how these can be improved.

Presentation Summary: Health Literacy affects patients' ability to access health services, interpret and act on health information. 60% of Australians have low health literacy. The Clinical Excellence Commission is working with LHDs and consumers to create a statewide framework to link, guide and support improvements in health literacy from systemic and individual perspectives.

Presentation Summary: Adopting a co-design approach, a Working Group of consumers, SESLHD staff and HCNSW staff co-designed the District's peak Consumer and Community Council (DCCC). The involvement of consumers with experience working with SESLHD has ensured the voice of the consumer and community was instrumental in the design of the DCCC.

Presentation Summary: Evidence of the patient experience is fundamental to creating effective health policy and service responses, yet we lack knowledge about their experience of adverse events (AEs). This presentation reports findings of a substantial project investing the experience of AEs from the perspectives of recently hospitalised patients in New South Wales.

Session Title: Unplanned readmissions within 28 days: The patient’s experience

Presentation Summary: Information regarding unplanned readmissions from the patients’ perspective is lacking in nursing literature, but vital to understand contributing factors. A mixed method research design with matched pair methodology was utilised. 114 patient interviews revealed issues regarding discharge information, medication information and referrals contributed to unplanned related readmissions or delayed discharges.

Session Title: "Real connection with a real person": Exploring what matters to people attending alcohol and other drug treatment

Presentation Summary: Person centred care in alcohol and other drug (AOD) treatment involves responding to what people accessing services say about what matters to them. This study involved conducting focus groups with service users and developing the patient reported experience measure for addiction treatment (PREMAT) based on focus group findings.

Presentation Summary: Patient experience survey data can provide a unique and needed perspective on obstacles to accessing care, which can occur at many points in the patient journey, such as knowing who to contact for care, travelling to services, finding healthcare services that you trust and feel respected.

In Australia organisational bodies reflect the importance of compassion as a core component to professionalism and cultural imperatives . This presentation will explore the facilitators and barriers that arise in the provision of compassionate care.

Session Title: Balancing caring for others and caring for ourselves: Soul time for staff

Presentation Summary: Support for staff and self-care needs to be reviewed and improved on a regular basis. In July 2017, Bear Cottage launched a 6 month “Soul Time” trial quality improvement project for staff and end-of-life care support. In the presentation we will explore the outcomes of our trial and our findings.

Presentation Summary: Undergraduate assistants in nursing (UGAIN) were introduced into the collaborative nursing model of a NSW regional hospital surgical ward. The UGAIN role provides direct patient care and has resulted in positive impact on inpatient experience and demonstrated improvements on inpatient safety with a reduction in fall and pressure injury rates.

Customised Mobile: Accessible discharge instructions so all patients leaving the emergency department know exactly What the Doctor Said

Presentation Summary: Patients leaving the emergency department do not understand their discharge instructions. Not understanding discharge instructions matters because of the higher risk of things going wrong at home after leaving the emergency department. What the Doctor Said is a web/mobile solution offering a robust, patient-centred and evidence-based approach to this issue.

Presentation Summary: Sydney Children’s Hospitals Network believes that all patients, families, and staff should feel safe and welcomed within the hospital environment – regardless of sexuality, gender, abilities, religion, or cultural beliefs. This workshop will examine and provide staff with informative ways to improve the patient experience of diverse population groups.

Presentation Summary: Aphasia is a common, lifelong consequence of brain injury. Aphasia is also a condition that suffers from low levels of public awareness. In 2016-17 The Aphasia Awareness Delegation worked in partnership with Speech Pathologists and Rotary to spread their frank but funny experiences of living with aphasia.

Patients in Emergency Departments were asked about their preferences in accessing information on pathology and medical imaging test results. Patients shared their perspectives surrounding context-sensitive health information technology providing tailored, patient-centred test results information.

We present our co-design journey with adolescent mental health inpatients to design and create a recovery mural for a sensory room. This gives our patients a sense of ownership of the space, creating a youth friendly environment.
