CPET Workshop & Prehabilitation Symposium Registration (Thursday & Friday) includes:
- Entry to all meeting sessions Thursday and Friday
- All catering breaks as per the program Thursday and Friday
- One (1) ticket to the Welcome Drinks Thursday Evening
- Entry to the trade display area
- Delegate Program

Perioperative Medicine Symposium Registration (Saturday and Sunday) includes:
- Entry to all meeting sessions Saturday and Sunday
- All catering breaks as per the program Saturday and Sunday

Day Registration includes:
- Entry to all meeting sessions on day of registration (Please refer to the program for details)
- Morning and Afternoon Teas and Lunch as per program on day of registration
- Entry to the trade display area on day of registration
- Delegate Program

Tickets for the Saturday Night Dinner are available for purchase during the registration process. Please note that dinner tickets need to be booked in advance and will not be available for purchase onsite at the event.


Alterations

Alterations to your registration will not be accepted over the telephone. Alterations can be made by logging back into your registration via the website. To access your registration you will need to enter both your email address and Registration Reference Number supplied at the time of the original registration. If you encounter any difficulties with the above please email reception@icms.com.au.

Payment

Full payment MUST be received prior to the Meeting(s) 2017. Registration cannot be confirmed until payment is received. Accommodation cannot be confirmed until credit card details are received. Please note if your registration is made less than two weeks prior to the Meeting(s) the only payment option available is credit card. Payment can be made in the following ways:


1. Credit Card Payment

All credit cards will incur a 3.25% service fee
Visa, MasterCard and AMEX are accepted


2. Cheque Payment

Please make cheque/bank draft Australian Dollars payable to “PoSH Meetings” and mail to the Meeting Office. International delegates please note: Cheque payment must be received by way of an Australian drawee bank. On receipt of your payment, confirmation will be emailed to you. It is advisable to check all items listed.


3. Electronic Funds Transfer

Please note that the Meeting will accept payments via electronic funds transfer. The Workshop bank details will be supplied on the invoice generated at the completion of your registration. Please include your Invoice number as a reference on the deposit and forward your remittance advice to the Workshop Office.

ACCOMMODATION INFORMATION

ICMS Travel, as the official housing bureau for the PoSH December Meetings, has been able to secure discounted accommodation rates at Pullman on the Park Melbourne. Reservations need to be made before
Friday, 3 November 2017. The room rate is per night and does not include breakfast. Accommodation will be allocated in order of receipt of bookings. For further information about The Larwill Studio Hotel please visit the CPET and EBPOM Workshop website.

Arrival Time

Check-in time is usually from 14:00. Should you wish to guarantee your room is ready before the hotel check-in time it is recommended that you book and pay for the room for the evening prior to your arrival. Failure to advise your arrival time may mean that your room will be released.

Deposit

To secure your reservation a deposit equivalent to one night's room tariff is required within 14 days of booking being made. Please ensure that valid credit card details are provided at the time of submitting your booking as these details are required to be passed onto the hotel as a guarantee. The hotel may not initially deduct a deposit payment from your credit card but may charge your full payment at the time of checkout. The hotel may charge a processing fee on payments by credit card. Bookings cannot be made until ICMS Travel receives your credit card details. Room rates are in Australian Dollars and include the Australian Goods and Services Tax (GST).
