Facility Service Agreement

Our mission is to provide a clean and welcoming campus by ensuring a quality environment for students, faculty, staff and visitors in support of Kaiser Permanente School of Allied Health Science’s mission of teaching and educating students.

This agreement is an agreement made between KPSAHS and requesting party for facility usage and services. 

The scope and service level expectations are as specified:

Outline of Roles and Responsibilities

  1. The Facilities Manager will serve as the main point of contact for all requests.
  2. All requests must be submitted via electronic submission using the Facility Request Form.
  3. By submitting a facility request you are agreeing to abide by the Facility Service Agreement.
  4. Notification in writing via email will be used for confirmation of requests.
  5. KPSAHS reserves the right to implement a change of agreement within five days of the requested services. This will include cancellation of services.
  6. A representative of the School will be on the premises for the duration of the requested time.

Service Commitments and Service Expectations

  1. Location venue will be the School's Lecture Hall unless otherwise specified.
  2. The use of the facility will not conflict with any schedule of the school, staff or students.
  3. The requestor must designate a person (Onsite Coordinator) to be responsible for members of its group.
  4. Requestors requiring special services and /or equipment must request specific arrangements in writing at the time of application.
  5. The facility must be left clean and in good order at the close of the contract period. The furniture and equipment must be returned to its original position.
  6. The requestor will have use to the parts of the building as approved and specified on the request form.
  7. Non-compliance with any of these rules and regulations may result in immediate cancellation of any future meeting request.
  8. If KPSHAS incurs any damage to equipment or to the facility.  It will be discussed with the designated point of contact and determination of liability will be determined by the provider of the damaged item(s) with the Facility Managers.  Liability issues will be reviewed and approved by Kaiser Permanente School of Allied Health Sciences Administration.
