FACULTY AND PLANNING COMMITTEE DISCLOSURE STATEMENT

Kaiser Permanente requires that all planners and faculty participating in CME activities disclose any financial relationship with a commercial interest that might pose a potential, apparent, or real conflict of interest with regard to the individual's contribution to the activity. A commercial interest is defined by the Accreditation Council for Continuing Medical Education (ACCME) as any entity producing, marketing, reselling, or distributing health care goods or services consumed by, or used on, patients; the most common examples are pharmaceutical and medical device companies. All potential conflicts of interest are mitigated prior to the start of the conference and in accordance with the policies of the KP National CME Program.
