Do you have a unique solution, intervention or strategy to improve case management? What tips and best practices could you share with case management and transitions of care professionals to implement into their practice? Share your knowledge at a future ACMA Chapter Conference by submitting your proposal to present. ​​​​​​​The deadline to submit your proposal is September 29 (11:59 p.m. CT).
