InfoWorld and Forrester Research, in collaboration with Penn State University Center for Enterprise Architecture, present the sixth annual Enterprise Architecture Awards, which recognizes companies whose practice of enterprise architecture has delivered substantial benefit helping their business address new challenges.

To be considered for the 2017 InfoWorld Enterprise Architecture Awards, send a nomination by completing the following form. To improve the nominee's chances of selection, please read the InfoWorld Enterprise Architecture Awards FAQ before submitting this nomination form.

Please be aware that only one entry can be submitted per person using this form. The questions listed each headings are suggestions of points you may wish to cover in your submission. Send any additional projects you would like to submit for consideration as a Word or Excel document to eric_knorr@infoworld.com. InfoWorld or Forrester may follow-up on individual submissions for additional information at our discretion.
