You will be asked to complete an evaluation form, this will be provided to you at the meeting and must be returned before you leave. Once you return the evaluation form you will be given the CME form. To receive CMEs for this course, you will need to fill out the form accordingly and fax it to the national office. You will then be emailed a certificate stating the CMEs received.
